What is the maximum amount of funding available, and are financial assistance funds paid directly to the family?

The maximum annual amount per individual is $3000.

Financial assistance funds will be paid directly to approved treatment providers, assessors, vendors or suppliers.​

You must complete and submit an application either via mail or drop off for assistance along with supporting documents.

In addition to the completed application, you will need to send the following documents:

  • Documentation of diagnosis
  • Description of request for assistance
  • Copy of previous year’s tax return
  • Letter(s) of recommendation (optional)
  • Proof of Fort Bend County Residency

Once we have received all components of the application (complete application form, doctor’s letter, and tax return, if applicable), your application will be reviewed within two weeks. The number of funds awarded per month would be based on funding available. Awards will not exceed $3000 anually per individual.

Yes, you may still apply even if you have health insurance. However, please note that our guidelines cover assistance for therapy, supplies, safety equipment, respite, and prescribed services. If your request is for something else, it will not be eligible. Feel free to contact our office if you have any questions.
